Community Partner Referral Form

The following referral form is for use by Out-Wayne County Community Partners that do not use HMIS and do not have direct access to the Out-Wayne homeless response system, Coordinated Entry but want to refer individuals/families experiencing homelessness to housing resources.  Assistance from the referring agency may be needed and is suggested in completing the form.  Referral signature is required.  

Please email the completed form to out-wayne_ces@waynemetro.org and a member of the Coordinated Entry team will contact the client within 3 business days of receipt of the referral.
